Office policies

What a visit costs
Your insurance plan sets your copay, and the office may not know the amount before your first visit. For current self-pay rates, call 352-672-6339. Self-pay patients may also request a written Good Faith Estimate before care.
Visits by video
Confirm virtual-visit coverage with your insurer before each appointment. If your plan does not cover the visit, you are responsible for the full self-pay rate.
New patient deposit
The deposit is $50 with insurance or $200 for self-pay. It holds your one-hour appointment and is applied to your copay or first-visit charge. It is refunded if insurance sets a $0 copay and forfeited if the appointment is missed or canceled late. Rescheduling may not be available.
Card on file
The practice keeps a card on file for balances and late-cancellation fees. If you prefer not to leave a card on file, you may instead leave a refundable $195 cash or check deposit. It is returned when your care ends.
Paying a balance
If you cannot pay a balance in full, call 352-672-6339. The office will arrange a payment plan based on what you can manage.
Appointment reminders
Patients are responsible for tracking their appointments. Reminders are a courtesy. Not receiving one does not waive the cancellation policy or fee.
Cancellations
Please give at least one business day’s notice to cancel. Late cancellations and missed appointments incur a nonrefundable $195 fee. Insurance cannot be billed for late-cancellation or missed-appointment fees. These fees are the patient’s full responsibility. This fee is separate from any deposit.
Arriving late
Arriving more than ten minutes late may mean you cannot be seen. The visit will be treated as a late cancellation.
Appointment length
New patient visits are 60 minutes. Follow-up visits are 20–30 minutes.
Prescription refills
Each prescription is written to last until your next appointment, so a refill should not normally be needed in between. If you run short, first ask your pharmacy whether any refills remain. If none remain, call 352-672-6339.
Records requests
Call 352-672-6339 to request medical records.
Services not provided
The practice does not complete disability paperwork or conduct workers’ compensation, disability, or other forensic evaluations.
