Becoming a patient

The practice is currently accepting new patients aged 18 and over. This is what happens before your first appointment.

Please provide complete and accurate insurance information before scheduling. If your coverage is inactive or the information is incorrect or incomplete, you are responsible for the resulting charges.

The process

How to become a patient

  1. New patient paperwork

    Call or email the office for intake forms, or download them from this website and return the completed forms by email.

    Download the new patient paperwork, complete it, and send the completed files by email to info@peacefulmindpsychiatry.com. If you are using insurance, please include photographs or copies of the front and back of your insurance card(s).

    The packet includes an optional consent for release of information, which authorizes the office to request your records from a previous provider.

  2. The office reviews your paperwork

    The office reviews your completed paperwork and verifies your insurance benefits, if applicable, before scheduling an appointment.

  3. The office calls you

    After reviewing your paperwork and verifying your insurance benefits, if applicable, the office will call you to discuss appointment availability and the new patient deposit. Deposits, fees and the card on file are all set out in the office policies.

    If you would prefer your first appointment to be by video, this is the call to say so. Whether a first appointment can be by video is decided case by case. It depends on what your insurance covers.

Before your first appointment

Please plan to arrive early. The office can be difficult to find. It is behind the Hunter’s Crossing Publix, in the Bristol Park complex, and there are directions to the door.

If your paperwork has already been completed and returned, there is nothing to bring with you. If any of it is still outstanding, bring the completed forms.

Your first appointment is 60 minutes with Dr. Liu.
